VIRTUAL CONSULTATIONS

Senior Steps new Offering

Caring for our loved ones while balancing other life responsibilities can be daunting and overwhelming. Senior Steps believes that having access to reliable and useful information in a timely manner can help alleviate those concerns.

 

Senior Steps is now offering Virtual Consultation and Assessment as part of our role as Geriatric Care Managers. With this Virtual consult, done over video-chat or other communication methods, we can provide a full-featured overview of a specific issue to be addressed or an upcoming problem that we can help rectify. These consultations are offered by our Geriatric Care Managers on staff, and can be scheduled directly on our website or by emailing consultations@seniorsteps.org or calling us at 617-405-8796.

 

Questions we can help with:

 

  • Can I modify my elder loved one's home so they can continue to age in place or is it time to look for a nursing home or live-in facility?

  • How do I make sure they are covered with reliable in-home care?

  • How do I plan for emergencies? What if something happens to them while I am out of town?

  • How do I handle care if I live far away? How can I make sure they get to their doctors' appointments and take their medication on time? 

  • How can we advocate for our relatives who may be in an assisted living facility or nursing home? What do I do if the level of provided care is not up to standard or if I suspect they are being abused?

  • How do I make sure my aging loved ones are living as healthy as possible? How can I help them socialize, be more active, and eat nutritious meals?

  • What do I do if my other family members and I cannot agree on how to handle management of care for an elderly loved one who cannot make their own decisions?

  • Are there steps I can take to automate or steamline some aspects of care to avoid burnout?

The practice, Senior Steps, Navigating Transitions in Later Life, provides guidance, advocacy, education, and coaching to secure the very best medical and home care for clients. How do we start?

We provide FREE 15 minute consultations over the phone or via e-mail to learn about your situation and determine what we can do to help. If you decide you would simply like a map or practical plan of action tailored to your elderly loved one, we conduct an in-person or virtual Care Consultation. A Care Consultation is an in-depth, personalized service for individuals and families who are facing many decisions and challenges associated with aging and related disorders. The goal is for each family to develop a better understanding of needs, make a plan to secure needed care, and develop strategies for the best possible care management and communication. A geriatric care management expert consucts a comprehensive assessment of what is needed immediately and can help you plan for care needs evolving over time. Consultations are provided in person, by phone, via video call, or through email.

Care Management is at the heart of what we do.

If you decide that you need more involved, long-term services, a team of geriatric care management experts will take over, partly or fully, depending on your elderly loved one's needs and wishes.

What is Care Management?

Care management is a set of activities intended to improve patient care and reduce the need for medical services by enhancing coordination of care, eliminating duplication, and helping patients and caregivers more effectively manage health conditions. These efforts have demonstrated potential to improve quality of life and control costs for patients.

What are the activities of Care Management?

Care management includes all aspects of monitoring and assisting in senior care. Medication review, discharge from hospital to rehab, rehab to home, advocacy, education, are just some of the aspects of Senior Steps Care Management. Our services go beyond health-related care and often includes regular visits to your elderly loved one for socialization, recommending trusted service providers (such as lawyers, in-home care providers, financial advisors, live-in facilities, and more). Reviewing medications, speaking with doctors and other healthcare professionals, and educating clients and family members to ask the right questions, empowers you to have full knowledge and understanding of appropriate care and treatment. Senior Steps makes sure you have control over your care!

How does Senior Steps Advocate? 

Senior Steps does NOT receive commissions from service providers like nursing homes or rehab facilities. We are only motivated by what is best for our clients. During transitions from home to a new living environment, or from the hospital to rehab, Senior Steps will advocate for the client's preferences to be respected and for the best quality of care to be provided. By monitoring medical, physical, emotional, and even social areas, Senior Steps will ensure optimum care and attention to client needs.
