We offer a Virtual Consultation with a certified Geriatric Care Manager to discuss and review Care Management options for you or an elder loved one. During the consultation we discuss: -Current health needs and care options -Financial resources and strategy -Living arrangements -Caregiver needs and requirements -Navigating the healthcare system -Assisted Living and Retirement Home options -Future prognosis and health planning -Any additional questions or concerns you may have. Our Care Managers work with each of our clients in a comprehensive and hands-on manner. Our Virtual Consult allows anyone with concerns or questions regarding Geriatric Care Management to have an in-depth, thorough conversation with a Certified Geriatric Care Manager prior to making any serious care and living situation decisions for an elder.
